Request for a Cost Analysis of Retirement Coverage In the Lump-Sum Division of the Statewide Volunteer Firefighter Plan (SVF Plan)

Participation in the SVF Plan is voluntary and open to fire departments as a replacement of their existing volunteer firefighter pension plan. The process for electing coverage of volunteer firefighters by the SVF Plan is initiated by a request for a cost analysis of the prospective retirement coverage using this form. Once completed, the form must be signed by representatives of the relief association and the governing body operating the fire department, and returned to the Public Employees Retirement Association (PERA). Transfer of coverage must be approved by the relief association and the governing body within 120 days of receipt of the cost analysis.
